JOB DESCRIPTION Village Health Group (VHG) is a friendly GP practice, across 4 sites within Rushcliffe: Keyworth, East Leake, Ruddington and Sutton Bonnington.Our friendly clinical team consists of 10 GP partners along with 7 salaried GPs who are supported by a team of highly skilled practice nurses, healthcare assistants and administrative staff. We are supported by community matrons, community COPD and heart failure nurses, and work alongside the district nursing and health visiting team. An on site 100hrs pharmacy provides extended pharmacy services. We have a cohesive, highly motivated team who are always looking to develop and improve our services to patients.

Job Title - Salaried GP Reports To -GP Partners Job Purpose Provide general medical services to the patients of the Village Health Group. Provide clinical assessment and deliver treatments to patients who have undifferentiated health care requirements in order to promote good health and prevent unnecessary acute hospital attendances.
Utilise advanced clinical skills and competencies in prescribing, health assessment, medicines management, leadership and case management Maximise patients health and reduce risks that contribute to ill health. Act as an autonomous practitioner who is open to the flexibility required for the role. Provide clinical support and advice to staff to promote caseload management techniques to reduce unplanned hospital admissions caused by poor control of long-term condition Work collaboratively across primary, secondary, social care and the voluntary sector to establish a whole systems approach to managing health care needs. Dimensions To work with the Village Health Group multidisciplinary team in provision of general medical services to residents and visitors of the practice population and their visitors.
Key Responsibilities Work with the Primary Healthcare Team on a day to day basis facilitating effective practice, efficient use of resources and smooth service delivery. Clinical Using expert knowledge, clinical skills and autonomous decision making skills, work collaboratively with other members of the primary health care team to provide a high standard of care. Comprehensively assess, review and evaluate the needs of patients with undifferentiated health care needs to improve their physical, psychological and mental health well being to promote self care and reduce the need for hospitalisation Accurately manage patients health within the practice, undertaking clinical assessment and providing the necessary treatment including prescribing using local and national guidelines as appropriate To develop specialist clinical skills (in self and others) to undertake interventions consistent with evidence based practice in line with national and local policy. Develop plans of care in conjunction with patients through: application of clinical knowledge and, analysis of symptoms and data, identification of risk factors associated with those conditions, recognition of early signs of acute illness, involving the patients and the carers in the care plan and ensuring it is understood, documenting safety netting plans and processes Use knowledge of medicines and prescribing skills to influence the reduction of risk and complications associated with medication and polypharmacy.
To utilise effective communication to aid onward referrals to specialists as appropriate. Facilitating a coordinated primary health care response to patient needs Ensuring effective communication channels and sharing of information that is beneficial to the patients care and avoids conflict of treatment prescribed by other professionals To work in partnership with patients, carers, GPs, and other health professionals as appropriate to promote, develop, initiate and implement proactive care. Link with existing services to facilitate care pathways and preventing hospital admissions where possible Act as a role model so that patients receive the most effective care possible through encouraging optimum management of healthcare conditions and acting in the patients interests at all times Encourage and empower patients and their carers in case management by providing accurate up-to-date information about disease prevention, progression and outcomes to embed a culture of self-management Highlight and report any Safeguarding concerns that may arise, working closely with the Safeguarding Lead where required Records accurate and contemporaneous I.T. based consultation notes to agreed standards using System One.
Organisational Contribute to the collection of appropriate data for QOF and other primary care DES - LES and other services Make, implement and communicate changes to clinical practice where necessary whilst continuing to work within scope of profession Ensure the safe management of care and service delivery, highlighting and reporting where necessary breaches of this (both internal and external) To work closely with other healthcare professionals, practice manager and Rushcliffe CCG to enable the development and implementation of care pathways in line with local and national guidelines Act as an educational resource to staff within the organisation Maintain up to date knowledge and competence in line with professional and service requirements and demonstrate critical thinking, decision making and reflective skills to analyse own personal and professional development Maintain confidentiality at all times Professional Maintains GMC registration and licence to practice Complete annual appraisals. Maintain a personal development plan and professional portfolio. Participate in the PRD review process to identify training and development needs. Remain updated and ensure that clinical practice is evidence based.
Communications & Relationships Internal Act as an integral part of the multidisciplinary primary health care team Disseminating information within the primary health care team Communicate effectively with other professionals and managers, sometimes with different agendas and priorities. The information that is communicated will be of a sensitive nature, as it will involve details of patients health, social status, including safeguarding concerns where identified. The communications will also involve complex issues around application of mental health capacity and Best Interest decisions Provide high quality written reports and any other written documentation as required. External Liaise with external agencies and professions effectively; this may be GPs, Secondary Care colleagues, Social Care colleagues, and voluntary organisations This job description is subject to periodic revision following discussion with the job holder.
