Here at Assist Home Care Ltd we are looking to recruit a Senior Care Coordinator to join our expanding team. We are looking for a committed, enthusiastic and caring individual to join our team of experienced care and support workers. We currently provide services to Adults and Children in Bradford, Kirklees and Calderdale.
The individual we are seeking must have extensive experience in a Care/Complex Care/ Health setting. Proven track record of operating at a senior level within the care sector, adhering to internal policies and procedures. Excellent communication skills, with a confident phone manner. Proven people management and coaching abilities, capable of motivating and guiding a team. Strong skills in rota management, with the ability to effectively address and resolve resourcing challenges.

Duties may include:
To provide care in accordance with current best practices, according to policy and procedures, agreed standards, legislative requirements, relevant regulations under the direction of the Care Manager
To supervise the care delivery within the service
Ensure Service Users are at the heart of the care delivery and their wishes and preferences enhance their wellbeing
To ensure that all staff contribute to the efficient running of and supervise staff
To organise and co-ordinate care delivery in accordance with current best practices, according to policy and procedures, agreed standards, legislative requirements, and relevant regulations under the direction of the Manager and within the financial plans
Ensure that the care service is delivered to Service Users at the times and for the duration specified in the relevant individual agreement(s) and contract standards
Develop effective working relationships with other employees within
Work in cooperation with members of the multi-disciplinary teams to maximise opportunities for Service Users
Monitor live calls using custom software and serve as the main contact for daily operational needs of our care team.
Make sure all necessary documents are in place for both current and new service users.
Help with the onboarding and training of new care workers and team members.
Take the lead in fostering a supportive and collaborative work environment for both the Office Team and your team of Carers.
Drive internal collaboration and effective work practices with the compliance, training, and recruitment teams.
Work closely with management to identify and address staffing risks, creating action plans as needed.
Serve as a point of escalation for office staff, confidently guiding them to find solutions.
Lead the implementation of new care packages, ensuring your team is fully supported to deliver the high level of service expected.
