Job Description


We are looking to recruit enthusiastic team leaders to join the Continuing Care Team.
The Continuing Care Team provides specialist support for children/young people with complex medical needs, in the community.
As carers we provide bespoke packages of care/support designed to meet the needs of the individual and primarily in their own homes.
As a carer within the Team, you will have access to trained Nurse support. You will receive competency based training in all aspects of care you will be required to deliver.
Role
To work alongside nursing/medical staff and family members to competently provide agreed clinical skills and individualised care for a child with complex health needs within the home, school and hospital environments, liaising with other professionals as necessary. To accurately record, collate, monitor and provide feedback of patient information to the Team Leader and/or Care Manager, initiating actions as necessary

Main duties of the job

  • Report any conditional changes of the child, which are causing concern to the parents/carer and/or trained nurse.
  • Recognise and respond appropriately to urgent and emergency situations.
  • Undertake transfer and escort of the dependent child if appropriate
  • Inform the trained nurse if the delegated tasks are beyond the post-holder's level of skill.
  • Contribute to the development of services and ensure the effective and efficient use of resources, notifying the Team Leader of any equipment difficulties.
  • Manage and maintain equipment as appropriate within Wirral NHS guidelines.
  • Participate in patient and public involvement activities.
  • Support people's equality, diversity and equal rights.
  • Provide client independence/self-care wherever possible - advocate in the best interest of the child at all times.
  • Be mindful and respectful of different lifestyles, cultures and religious beliefs.
