The post holder will be employed by South & East Leeds GP Group, but will be deployed and managed day to day from within the Beeston, Middleton & Hunslet Primary Care Network Outline of the post: Beeston, Middleton & Hunslet PCN is seeking a Senior Occupational Therapist to be part of our multi-disciplinary and multi-agency team of professionals. We are looking for an individual who is passionate about Primary Care and is keen to work across traditional health and social care boundaries.
This is open to anyone with relevant registration. This is an exciting and unique opportunity to be at the forefront of the service delivery of our PCN Proactive Care Team. Working both independently, and within our multi agency team, the role will operate across the traditional health and social care organisational boundaries. This includes our GP practices and will deliver enhanced proactive care for older people with frailty.
We are looking for a Senior Occupational Therapist with a strong interest in working in Primary Care and who aspires to provide a continuous improvement approach to delivering responsive, safe, patient-centred, and effective care within a team. The service, within which the post holder would work, supports patients with multifaceted health problems living in their own homes. The service liaises with carers, families, and colleagues to maintain and maximise individualised patient-centred care. It also supports General Practice in delivering exciting new developments and this can be flexible depending on skills, expertise, and interests.
Main Tasks:
Clinical Deliver expert knowledge, advanced clinical reasoning and care planning skills to the multi-disciplinary team. To use advanced knowledge and practical experience to supervise, co-ordinate and manage the proactive care planning of the designated patient cohorts To liaise with other health and social care professionals and agencies to ensure holistic care is provided and the care packages offered are appropriate and consistent with the patients needs and aspirations. To ensure that the patients and their families and carers are fully involved in the assessment and care planning process. To ensure best practice relating to care planning and the management of frailty and long-term conditions is adhered to Managerial Responsibilities: To co-ordinate, monitor and assess the work of the PCN Proactive Care Team Provide Clinical Supervision to the PCN Proactive Care Team on a regular basis To provide emotional support for team members in potentially stressful work situations or adverse incidents To identify any service improvements or efficiencies Be involved in service development To contribute to the staff recruitment process To complete return to work interviews with staff following sickness in line with the Sickness Absence policy Health and Safety - The post-holder will assist in promoting and maintaining their own and others health, safety and security as defined in our Health & Safety Policy, to include: The post-holder will manage their own and others health & safety and infection control as defined in the SEL GP Group Health & Safety Policy, the SEL GP Group Health & Safety Manual, and the SEL GP Group Infection Control Policy and published procedures.
Comply with SEL GP Group health and safety policies by following agreed safe working procedures Awareness and compliance with national standards of infection control, hygiene, regulatory / contractual / professional requirements, and good practice guidelines. Correct personal use of Personal Protective Equipment (PPE) and ensuring correct use of PPE by others, advising on appropriate circumstances for use by clinicians, staff and patients. Undertake mandatory and statutory training as required and attend courses/ study days as deemed appropriate/ necessary. To demonstrate a commitment to personal and professional development. Keep updated on developments in care planning and the management of frailty and long-term conditions and consider how to incorporate these into practice. Work with other advanced clinical practitioners to ensure the provision of an appropriate and relevant TARGET training programme for the team and support staff to develop their knowledge and skills.
To lead by example and provide on the job training and development for less experienced members of staff. To be responsible for maintaining own competence to practice through CPD activity and maintaining a portfolio that reflects personal development. To participate in annual appraisals as an appraiser and appraise. To represent the team at a strategic level at the PCN leadership meeting (upon request) To be an integral part of the Community PCN leadership team Governance Produce complete and accurate records of patient consultation, in line with best practice, confidentiality, policies and procedures Deliver care according to NHS guidance, NICE guidelines and evidence-based care Take part in the maintenance of quality governance systems and processes across the PCN and its activities.
Utilise the audit cycle as a means of evaluating the quality of the work of self and the team, implementing improvements where required. Work with other clinical teams on improving the quality of healthcare in response to local and national policies and initiatives as appropriate Evaluate patients response to health care provision and the effectiveness of care Support and participate in shared learning across the practice and wider organisation Manage, review and identify learning from patient complaints, clinical incidents and near-miss events Awareness of statutory safeguarding, notification processes and local guidance for children/vulnerable patients, applying relevant policies and legislation to protect them Ensure compliance with policies, procedures and guidelines for self and others, by taking action or alerting senior management team if the practice appears to contravene policy, or if there are concerns over any aspect of patient care. Confidentiality: Maintain confidentiality of information, always acting within the terms of the Data Protection Act and Caldicott guidance on patient confidentiality. Maintain an awareness of the Freedom of Information Act.
