We are seeking a compassionate and dedicated Support Worker to join our team. The ideal candidate will play a vital role in providing high-quality care and support to individuals in various settings, including group homes, assisted living facilities, and private residences. This position requires a commitment to enhancing the quality of life for those we serve, particularly individuals with developmental disabilities, Alzheimer's disease, or other memory-related conditions.
Duties
Assist clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and meal preparation.
Provide patient care and support in accordance with individualized care plans.
Monitor and document vital signs and changes in clients' conditions.
Assist with medication administration as per prescribed guidelines.
Engage clients in meaningful activities to promote social interaction and cognitive stimulation.
Maintain a clean and safe environment through regular cleaning and organization of living spaces.
Support clients with mobility needs, including the use of Hoyer lifts or other assistive devices.
Collaborate with healthcare professionals to ensure comprehensive care for clients.
Uphold HIPAA regulations to maintain client confidentiality and privacy.
Provide companionship and emotional support to enhance clients' well-being.
Qualifications
Previous experience in home health care, senior care, or long-term care settings is preferred.
Familiarity with patient observation techniques and monitoring vital signs.
Knowledge of caregiving practices for individuals with disabilities, including autism and dementia care.
Certification in First Aid and CPR is highly desirable.
Strong communication skills to effectively interact with clients, families, and team members.
Ability to perform heavy lifting as required by client needs.
Experience working in group homes or nursing facilities is a plus.
Understanding of care plans and the ability to implement them effectively.
