Job Purpose



To successfully lead young people who are placed with Smile Care Services (SCS) towards independence by setting them SMART objectives and supporting them to achieve these

Requirements of Role



To work with young people to ensure activities are taking place to deliver the Achievement Plan / Pathway Plan To encourage young people to undertake the activities on the Weekly Planner o To ensure keyworking sessions take place and keyworking forms are properly completed to reflect and record what has been done To support Key Workers to create weekly feedback reports by ensuring that all relevant paperwork (contact records, keyworking forms etc) has been completed and to a high standard To ensure the production of reports and correspondence required are to a high professional standard Ensuring general up keep of the house is to a high standard
To read the risk assessment at the beginning of every shift to ensure full awareness of risks associated with the young person at the start of every shift o

To work jointly with young people in assessing their needs, preparing, implementing, delivering and revising Achievement Plans / Pathway Plans and Weekly Planners

To ensure that the physical, cultural, emotional, religious, educational and psychological needs of young people are respected and met To apply relevant de-escalation / distraction techniques to prevent young people from engaging in challenging behaviour / offending activities
To attend appointments and meetings and to contribute information to reports on young people as requested

To respect and enforce professional boundaries and communication with young people, colleagues, professionals from statutory or voluntary sectors and other agencies o To encourage young people to have a voice and input into the service they receive and to follow up on this accordingly
Job Type: Full-time

Pay: From 13.00 per hour

Schedule:

8 hour shift Day shift Holidays Night shift
