Rock Court in Sutton un Ashfield require a Senior Support Worker - full time position including some weekends, being on call and sleep ins

Responsibilities



1) To ensure a flexible approach from the team to meet the needs of the individual.

2) To ensure that the individual support plans of each person are adhered to and reviewed constantly by the staff team.

3) To co-operate with the individual in developing their person-centred support plan.

4) To support individuals and to actively participate in the support planning process, as acting as keyworker.

5) To assist the Register Manager in ensuring that the support being delivered is Effective, Responsive, Safe, Caring and Well-led in line with the CQC Quality Statements.

6) To liaise with family and other professionals involved in the health and well-being of the individual being supported.

7) To assist the Registered Manager in any incidence, suspicion or sign of abuse of an individual. Or any other matter pertaining to the protection of Adult at Risk in line with local and Sun Healthcare Safeguarding Policy and Procedures.

8) Respect the equality & diversity of your colleagues and the people you support.

9) To report and record any accidents, complaints or untoward incidents to the appropriate personnel and be aware of the critical incident procedure.

10) To ensure that individuals are supported in maintaining their tenancy and liaising with the landlord.

11) To liaise with the Asst. Manager on issues affecting the service eg any shortfall in staff cover.

12) Flexibility to carry out waking night duties/sleep ins if required.

13) To assist in the devising and management of the staff rotas & take part in the on-call rota.

14) Administer and record accurately all medication given. Order, store and record all medication received, and assist in carrying out medication audits.

15) In liaison with the relevant Service Manager ensure that individual risk assessments/management plans are carried out and that appropriate strategies are in place for those individuals we provide support to.

16) To assist staff and support individuals with budget planning, paying bills and shopping.

17) To ensure clear accurate financial records are kept for each individual we support.

18) To assist staff to support individuals in maintaining a safe and secure home.

19) To be aware of relevant legislation i.e Care Quality Commission, Data Protection and comply with all regulations. and be aware of requirements of any Quality Assurance Framework Standards.

20) To be proactive in achieving external quality accreditation as and when relevant.

21) To carry out and record formal, observational and other necessary one-to-one supervisions and appraisals as well as actively participating in group supervisions and meetings.

22) To attend supervision and appraisal sessions To ensure that all relevant E-learning training remains as a minimum requirement at 90%. Also, completion of the Care Certificate within 12 weeks of commencement of role (unless an equivalent or higher qualification in a relevant field has been completed previously). Also attend identified training courses to further knowledge and understanding of learning disabilities / complex needs and all aspects of job role and any training deemed necessary by the Registered Manager.

23) To ensure the staff team are aware of the Grievance and Disciplinary procedure.

24) To ensure, as far as reasonably practicable, observance and adherence to the Health & Safety at Work Act, in particular, to take all necessary steps to ensure the personal safety of individuals you support, staff and yourself.

25) Carry out any other duties that may be reasonably expected of a Senior Support Worker at the discretion of the Registered Manager
