Assist with rehabilitation and reablement plans
Lead shifts and support junior staff
Monitor progress and maintain care records
Support discharge planning and handovers
What We're Looking For
Essential:
Experience in a senior care or support worker role
Strong communication and leadership skills
Confidence using digital care records
Desirable:
Level 3 Health & Social Care qualification
Experience in rehab, reablement or intermediate care
Experience using Nourish care software
