YPEP is looking for a Care Coordinator to join our team In Pontefract/Castleford
Are you a dynamic person with experience in planning, problem solving, rostering with a can do attitude, then you click the apply button.
You will be working with the team to ensure that the business and compliance met the target.
Key responsibilities:
Allocating right skilled and experienced care workers to deliver the best care as per the needs of the service user. Ensure the service users are at the heart of care delivery. You will be organizing the rota on the system Playing a key role in getting new contracts. Ensuring the assessment is done and the care plan is finished on time. Must ensure the documentation is in situ and completed for each service user. Maintaining a strong relationship with the key stakeholders which includes Social workers. District nurses , Safeguarding team, commissioning teams, Occupational therapist and PT's . On call duties as and when required. To achieve targets, KPI's and objectives set by the Registered Manager. Ensure all staffs are met the compliance as required by CQC
We looking for :
At least 2 years of experience as domiciliary care coordinator Able to work as a team player and must have the to do mind. Excellent organisational skills, communication skills both written and verbal. IT knowledge : Microsoft office. Driving license Experience is handling complaints and problem solving skills.
