to join our team.You will join us on a

fixed term contract until 31st MARCH 2026

, and in return, you will receive a salary of

19,218.40 pro-rata per annum (FTE 24,023 per annum). Home based with travel across Leicester, Leicestershire & Rutland.

Overview




The Home from Hospital service offers support to patients who are awaiting / have recently been discharged from hospital. Support commences upon notification of discharge. Patients will be supported to leave hospital though transport from hospital to their home (provided by staff/volunteers via their own vehicles).


Once home, the support continues for 6-12 weeks, helping people reconnect with their community or activities of interest. The service aims to cut delays in patients being able to leave hospital when they are ready, and to reduce their recovery time and chance of readmission by supporting their wellbeing during their first days at home. Our support helps to reduce the impact of isolation and loneliness and enable people to be better connected with others for their ongoing wellbeing.


We're currently looking for two

Service Co-ordinators

to join our team on a part-time basis, on a

fixed-term contract until 31st March 2026.




The Role




The purpose of the Service Coordinator is to support our volunteers and clients, working closely with the Service Manager to deliver our Home from Hospital service. Ensuring the day-to-day effective coordination of volunteers and capturing all information in our database. Recruitment, training and supporting volunteers, receiving, and actioning referrals. Delivering our service within the line of our funding agreement and Royal Voluntary Service vision and core values. Applicants must have access to own vehicle.



Working Hours 28 hours




Monday to Thursday - role 1



Tuesday to Friday - role 2





What you'll need:




Knowledge




A working knowledge and understanding of the NHS and Health and Social Care including the needs of patients upon discharge from hospital. Awareness of the principles of Safeguarding (Adults and Children), (training and ongoing support is provided.) Understanding of the strengths and support needs of a volunteer involving organisation and volunteers. Ability to proficiently use standard IT systems / Email / Microsoft Teams and learn role specific databases.

Skills




Excellent communications ability at all levels. Ability to act on own initiative within agreed parameters. Willingness to transport patients, both home from hospital and to other identified community settings. Full UK driving licence and access to own vehicle. The ability to regularly travel across a defined geographical area to support patients discharged from hospital.

Experience




Demonstrable experience of working in a person-centred manner with care, compassion and sensitivity. Demonstrable ability to support, calm, enthuse and assess risk in all occasions.
