About The Role
Service Coordinator - Assisted Discharge - Homelessness
Location: St Thomas Hospital covering the London boroughs of Lambeth, Southwark and Lewisham
Contract: Fixed term Util 31st March 2026, possibility of extension
Hours: 35hrs per week, full-time Monday to Friday 9am to 5pm
Salary: 26,719 per annum including (plus London Weighting allowance of 3,344 if applicable))

The British Red Cross are looking for an enthusiastic and proactive individual to join our Health and Care team as a Service Coordinator. This exciting position is for someone who has excellent interpersonal and communication skills and is able to lead and motivate people.
A Day In the Life of a Service Coordinator will include

  • You will respond to referrals from the King`s Homeless Pathway and Adult Social Services and will be required to have the ability to assess service users' needs, complete and follow a support plan for the individual.
  • You will work closely with another British Red Cross Support Worker and the hospital Homeless Pathway Team to ensure homeless patients have sufficient support and provisions and they are connected with statutory and voluntary services in the community after hospital discharge.
  • You will be responsible to follow patients up after discharge as necessary, including visiting them in their discharge location as required, escorting patients to appointments and liaising with other agencies to achieve the best results for them.
  • You will be responsible to support patients who have been discharged into temporarily hotel beds in Peckham, Crystal Palace and Croydon area. Those homeless patients are individuals eligible for housing support but are waiting for housing allocation from local authorities. Responsibility to carry out holistic assessments that will help the Homeless Team to complete a support plan for those individuals.
  • Thorough records will be required to evaluate and analyse the outcomes of the services, and this will be the responsibility of the Service Co-ordinator.
  • Assigning incoming referrals to a Support Worker, monitoring the service delivery, ensuring service health and safety, evaluating progress, overseeing record-keeping, engaging and managing volunteers, attending hospital meetings, and ensuring the team works together seamlessly.
  • Acting as contingency for Support Worker when required and may need to provide cover for their colleagues at other hospitals served by the Red Cross in the area. Service confidentiality and safeguarding vulnerable people are key responsibilities of the role.
This does not include personal care and management of medication.
To be a successful Service Coordinator you'll need
To be able to demonstrate experience of working with vulnerable adults. * To be a good communicator who can delegate and manage set tasks to agreed timescales.
  • Experience preparing and keeping written records and reports.
  • The ability to work with NHS professionals, community organisations, care agencies and relatives in order to support people in the community to life safely and supported at home.
  • A Level 3 qualification in care or currently be working towards this will be an advantage.
