What's the role all about?





As a Social Care Coordinator for Education, Health and Care Plans (EHCPs), you'll be responsible for ensuring the social care elements of EHCPs are robust, timely, and aligned with national standards. You'll work on behalf of children and young people aged 0-25, whether they are open to social work services and ensure that their care needs are accurately reflected in their plans. Your work will help the local authority meet its statutory responsibilities under the SEND Code of Practice.


What will your day look like?





This role is all about bringing people together and making a real difference. You'll be at the heart of coordinating and completing the social care elements of EHCPs, working closely with families, social workers, and a range of professionals to ensure everything runs smoothly. From attending reviews and multi-agency meetings to making sure the child's voice and family needs are front and centre, your input will be vital. You'll keep records sharp and up to date, stay on top of the latest legislation, and be a go-to source of support and guidance for colleagues navigating the EHCP process.
