We have an exciting opportunity for a Social Care Practitioner to join the Initial Contact Team in Adult Care. The successful candidate will join a friendly, supportive and dynamic team. We are committed to providing good quality supervision and continuing professional development opportunities.


Your Next role




The Initial Contact Team is responsible for delivering a high quality and responsive service through strength-based conversation, to understand and where possible resolve queries and issues raised at first point of contact into Adult Social Care. The Initial Contact Team offers short term involvement and by using a strengths based approach we signpost, explore all community based options and assess and arrange short term and long term support.


As a Social Care Practitioner, you will be supporting residents to remain independent in their own homes with access to effective information and advice and services that promote a strength base approach.


You will demonstrate good communication skills and enjoy working in a fast pace environment. We are interested in applicants who have experience of and a passion to work with adults and are committed to providing a person centred and strengths based approach to their work. The role entails working in partnership with adults and carers to assess need, set up outcome focussed support plans as appropriate as per the requirements of the Care Act 2014.


You will need experience working with adults with complex needs. You should hold an NVQ Level 3 in Health and Social Care (or equivalent. You must understand relevant legislation and the ability to work independently and manage a varied caseload.

Candidates must be fluent in the English language (as a requirement of Part 7 of the Immigration Act for the effective performance of a customer-facing role).


A valid driving licence and access to a car is essential for this role.
