About the Role

We're looking for an enthusiastic and compassionate Social Prescriber & Health & Wellbeing Coach (Mental Health Focus) to join our expanding multidisciplinary team within St Helens South Primary Care Network (PCN).



This exciting role combines social prescribing and health coaching to provide holistic, person-centred support for individuals with mild to moderate mental health needs. You will work with patients to identify personal goals, promote self-management, and connect them to community-based support and activities that improve emotional resilience, social connection, and wellbeing.



You will also triage referrals to support the Senior Mental Health ANP, ensuring patients are directed to the most appropriate pathway quickly and safely.




What You'll Do




Provide one-to-one coaching and social prescribing support to patients referred by GPs, ANPs, and other PCN staff. Triage referrals for the Senior Mental Health ANP -- perform initial assessment of referral appropriateness, prioritisation, and onward signposting to ensure timely and safe care pathways. Use motivational interviewing and health coaching techniques to help people set achievable goals and make positive lifestyle and mindset changes. Support patients in accessing community groups, voluntary organisations, and local mental health resources. Work closely with the Senior Mental Health ANP to deliver wraparound care, ensuring non-clinical interventions complement clinical treatment. Conduct wellbeing checks, follow-up reviews, and monitor progress to help patients build confidence and independence. Collaborate with PCN colleagues including Care Coordinators, Mental Health Practitioners, Social Prescribers, and GPs to deliver joined-up, person-centred care. Contribute to the ongoing development of the PCN's mental health and wellbeing service.



About You




Have experience supporting individuals with mental health needs, social isolation, or complex life challenges. Are skilled in motivational interviewing, coaching, or behaviour change approaches. Have experience or aptitude for triage, risk recognition, and making appropriate onward referrals. Are empathetic, patient, and able to build trusting relationships with people from all walks of life. Can work independently while contributing to a wider multidisciplinary team. Are passionate about improving mental wellbeing and reducing health inequalities in your community. Experience in health, social care, or community settings is desirable -- but full training, induction, and professional supervision will be provided
