Job Purpose:



To coordinate delivery of the social prescribing service

Location



Based across East Kent as required. Main Red Zebra office in Whitstable.

Hours



Part time, 22.5 hours per week.

Annual Salary:



24,500 pro rata, rising to 25,000 after successful probation period (3 months)

Annual Leave:



25 days per annum, plus bank holidays (pro rata if part time)

Who are we looking for?



You must be an enthusiastic team player who is a creative problem solver, someone who demonstrates initiative and has a positive 'can do' attitude. A willingness to get on and succeed is an absolute must. You must have excellent communication skills, with the ability to talk to people of all ages on any level in a variety of settings, including 1-2-1 meetings, small and large presentations to organisations and GPs and communications with many other health care professionals. This role is extremely proactive and you will be working to agreed targets but being a "self-starter" with the ability to work alone is a must. You will work with a great close knit team and being a good team player is essential with this friendly bunch. We have a community focus, so experience in a similar setting would be great but is not essential.

Purpose of the role:



The successful candidate will be partly responsible for the operational delivery of the community based Social Prescribing service, receiving referrals from a range of referrers including GPs and other Healthcare professionals in the East Kent Area. The role supports individuals to access a range of health, social care, welfare advice and other support services with the aim of improving their physical & mental health and overall wellbeing.

The role will require proactive engagement with other professionals and patients to stimulate referrals into the programme and with community-based providers of services and activities. Individuals will be supported on a person centred basis to identify activities that will improve their health and wellbeing. Building great relationships and enhancing our existing connections with other organisations is key to connecting people to the social activities they want and need.

Responsibilities and duties:



Delivery



Act as point of contact, and independently manage referrals from staff within Red Zebra as well as external enquiries Provide information to clients, including signposting, referring and "warm handovers" where appropriate Work with individuals, and in some cases their families, in a supportive, holistic way to address practical and psychological barriers, coping strategies for daily living, low confidence and social isolation; work together to co-produce a solution Update and maintain patient database and other client records through working with health professionals to deliver feedback and monitor outcomes Facilitate the sharing of information, resources and intelligence around gaps in provision Develop a comprehensive wide knowledge of support services in the community to meet different needs including: social isolation, long term health physical and mental health conditions, wellbeing, etc Where appropriate, set up and coordinate special events and outreach in the community Attend MDT meetings as appropriate Work with wider team to increase awareness of the benefits of social prescribing Promotion of positive mental health and prevention of mental health issues.

Monitoring, reporting and evaluation



Monitor and evaluate the effectiveness of the services against agreed targets Maintain system to keep accurate records relating to the delivery of the social prescribing project Produce and submit service reports when requested in agreed format Undertake regular evaluation of the service ensuring that the clients are at the heart of the process

External partners



Maintain effective relationships with health and social care professionals, information, advice and guidance services, support services, stakeholders and partners and community groups Maintain good communication with clients and their families and carers Recognise people's needs for different methods of communications and respond accordingly

Qualifications



Essential



English & Maths GCSE grade A-C

Desirable



NVQ level 3 in health and social care

Experience



Essential



Personal or professional experience in working with individuals struggling within the health, education or VCSE sector Experience of developing and maintaining relationships with service users Proven experience of working with a range of health, social care and VCSE professionals Ability to act on own initiative and organise personal workload effectively Experience of meeting deadlines and targets

Previous experience of working with individuals to identify and action personal goals Experience of developing or delivering community based projects
