To support the PCN with pro-actively assess the needs of their population and identify patients who would benefit from social prescribing support - E.g. carers, people recently bereaved, people with unhealthy lifestyle choices and those suffering with loneliness and isolation. To provide personalised, practical support to patients, their families and carers enabling them to make decisions that will improve their health and well-being outcomes and to increase their independent living and support by connecting them to
community initiatives for sustainability.
Take a holistic approach, based on the person's priorities and the wider determinants of health using coaching techniques and a develop trusting relationships by giving people time and focus on 'what matters to me' and co-produce a personalised care
and support plan.
The role will require managing and prioritising your own caseload in accordance with the needs, priorities and support required by individuals on the caseload. It is therefore vital that the Social Prescriber has a strong awareness and understanding
of when it is appropriate or necessary to refer people on to other health professionals/agencies, when the person's needs are beyond the scope of the role -e.g. when there is a mental health need requiring a qualified practitioner.
