The Social Prescribing Link Worker empowers individuals to improve their health and wellbeing through personalised, non-medical support. Based within the Brownlow Primary Care Network (PCN) and local GP practices, the Social Prescribing Link Worker connects people to community groups, voluntary organisations, and services to address issues like loneliness, housing, debt, and inactivity. They will help develop the The Social Prescribing Link service, support PCN priorities, and collaborate with partners to sustain or create local initiatives. This role is especially valuable for people with long-term conditions, mental health needs, social isolation, or complex social challenges.
The Social Prescribing Link Worker will manage their own caseload, promote lifestyle changes, and work closely with a multidisciplinary team. They will receive monthly clinical supervision and have access to support for safeguarding, domestic violence, and mental health concerns. GP referrals will be recorded using SNOMED CT codes in clinical systems.
