Key Work Areas

: To lead and manage teams delivering a range of social prescribing services working within specific NHS contracts. To manage the service deliverables and performance required within each, ensuring accurate recording and reporting of client data and outcomes.

To be accountable for senior level reporting and partnership management with Integrated Care Board colleagues and working with team leads to maintain the quality and effectiveness of our services in line with ICB standards and the Quality of Advice standards set by Citizens Advice.

To manage an active programme of community engagement, based on the identification of need and interactions within the Ipswich East and West Integrated Neighbour Teams. To represent Citizens Advice Ipswich by attending events and advocating for the services we provide.

To develop and refine our social prescribing services (and supporting processes) delivered by Citizens Advice Ipswich, in line with changing funder and/or client demand and technological opportunity.

To produce high quality Management Information around service demand, trends and insights, including monthly and quarterly performance reports.

To be responsible for the management and development of the Social Prescribing team training programme - based on national Citizens Advice guidance and Social Prescribing service standards. To provide a structured framework which supports progression and the identification of training need or knowledge gaps.

Key Responsibilities



Daily Team Management



Support the social prescribing team leads and/or senior advisers (within a range of social prescribing contracts) across all aspects of operational team, workflow, process and performance management. Undertake regular 1 to 1 meetings with social prescribing team leads around performance and quality of service, providing support and coaching to build and grow team lead competencies and confidence. Drive and co-ordinate team performance measures to ensure contract deliverables are achieved, that system, people or process issues are highlighted and to identify timely, prioritised actions to remedy.

Performance Management



To interpret operational data to produce monthly and quarterly social prescribing performance reports; that aid and inform resource planning, training and future service considerations. To develop and maintain a social prescribing dashboard to articulate performance and service delivery metrics. Participate in business, team and partnership meetings to share and inform around social prescribing performance. Proactively encourage teamwork across departments, facilitating good lines of communication between all members of staff.

Social Prescribing Service Development



Remain up to date and informed around changing technologies and advice practices - as applied through the national Citizens Advice model and/or national social prescribing standards. Seek opportunities to improve and refine the Ipswich social prescribing service and operating model.

Other Duties and Responsibilities



Carry out other tasks, which may be within the scope of the post, to ensure the effective delivery of social prescribing advice and support services. Work effectively with senior management team peers to support wider operational processes, or the collaborative delivery of activities or projects.
