Adults Hospital Discharge Social Worker

Locum

38 an Hour

3 Month Initial Contract

Hampshire County Council are recruiting for a Social Worker in their Hospital Discharge Teams based out of Queen Alexander Hospital (Portsmouth) with adhoc visit required at Royal Surrey Hospital.

Responsibilities:

Working with the acute team. The focus will be on supporting the turn around of patients at the front door and working with the Single Point of access here at the hospital. This role requires you to be on site 4 days a week as you will be assessing patients for discharge planning. 1 day a week will be working remote covering another hospital.
