28,000 to 30,000 per annum + Mileage Reimbursement at 0.25 per mile
We are seeking an experienced and compassionate
Specialist Care Coordinator
to support a young male client with a complex healthcare condition.
This is a unique opportunity to deliver high-quality, person-centred care by working exclusively with one individual, ensuring a truly bespoke and consistent service.
As a Specialist Care Coordinator
you will:
Support and Manage
: Lead our Peripatetic Care Assistants, ensuring their effectiveness in delivering care. including rota coordination and day-to-day management.
Collaborate
: Work closely with our Care Managers to manage care packages and uphold the highest standards.
Ensure Safety
: Support our clinical team in maintaining staff training and competencies to ensure client safety.
Travel
: Travel to the client's location must be achievable within 30 minutes from your home.
On call:
be part of the on call on a rota basis
24 hours a week working on shift as part of the Complex Care Team* 16hours rota and staff management*
About You
You will be a
Specialist Care Coordinator
, that thrives on problem-solving and enjoy being part of a fast-paced team, while maintaining a methodical attention to detail. This role requires you to be flexible and adaptable, able to amend plans at short notice.
Your Requirements
Experience
:
A background in health and social care is essential.
Experience with complex clinical care is highly advantageous
Experience managing sensitive or difficult conversations and a proven ability to build strong, positive relationships with both staff and clients.
Skills
:
Proficiency in rota planning, effective communication, and the ability to multitask
Qualifications
:
Hold at least NVQ Level 3 in care with a willingness to complete a management and leadership course.
A full UK driving licence is essential, and you must be confident in driving both the client and staff as needed.
Travel to the client's location must be achievable within 30 minutes from your home
You must also be willing to learn and undertake relevant clinical tasks, complete a minimum of two care shifts per week as part of the regular rota, and provide ad hoc shift cover when required, sometimes at short notice
