About us

------------

This appointment is part of the SPROCKET Hub (Systems And Process Redesign And Optimisation At Childhood Key Events And Transition. SPROCKET is a multidisciplinary Hub which brings together experts in operational research, systems thinking, health services research, clinical paediatric practice, innovation implementation and evaluation; design of interventions for people with disabilities; community facilitators, parents of children with complex health needs; special educational needs experts and experts in co-production and health inequalities.

Our aim is to transform the delivery of services for children and young people with complex health needs. Children and young people (CYP) with complex health needs, and those that care for them, face significant difficulties navigating services across health, education, and social care, leading to feelings of being overwhelmed, isolated, and burdened. Key transitions, such as moving between services or starting a new school, are particularly challenging due to barriers like poor communication, stigma, and lack of funding and resources. As a result, there are widening inequalities in access to and experience of care, and increased risk of adverse health outcomes.

Whilst initiatives are ongoing to improve transitions from children to adult health services, there are many other significant transitions that are often overlooked. The SPROCKET Innovation Hub aims to address these gaps through working to improve two transitions: hospital-to-home and education transitions, with a focus on equity.
