Located in the heart of West Sussex, The Apuldram Centre provides a variety of services and activities including Supported Living, meaningful day opportunities, and community engagement. Job Purpose:
As part of our dedicated team, you will play a key role in making a real difference in the lives of our service users, helping them grow and thrive both personally and within the wider community
You will assist individuals with learning disabilities in their homes with personal care and support needs. You will work collaboratively with individuals to promote their independence, encourage choice, participation, and motivation, while consistently upholding the values of the organisation.
This role provides a varied and community-oriented experience, as you help individuals navigate daily tasks, social interactions, and personal development within the context of their everyday lives.
Key Responsibilities
Provide Personalised Support:
Assist service user with personal care and daily living tasks as outlined in their individual support plans, developed in collaboration with them, their families, and professionals.
Respect Individual Preferences:
Ensure service users have opportunities to express their preferences and participate in decisions about how their support is provided, promoting dignity and choice.
Maintain Effective Communication:
Collaborate with service users, their relatives, friends, and other staff members to ensure continuity of care and high-quality support, fostering a safe and respectful environment.
Report Changes in Condition:
Regularly communicate with House Coordinators, Team Leaders, and the Supported Living Manager, especially regarding any changes in the service user's condition or circumstances.
Promote Independence:
Encourage service users to maintain and enhance their independence, taking into account their physical, emotional, and social needs, while actively engaging them in the running of their home and daily activities.
Record Accurate Documentation:
Keep detailed, up-to-date records in the organisation's electronic care management platform, Log My Care, including support diaries, MAR charts, daily checklists, handovers and other relevant documentation.
Support Community Engagement:
Assist service users to access local facilities and participate in recreational, educational, social, and leisure opportunities to promote inclusion and well-being.
Develop Life Skills:
Encourage and assist service users in developing domestic skills, such as meal preparation, making healthy choices, personal hygiene, and maintaining their living space. Paying particular attention to tasks like clothing care, arranging visits with family, and coordinating leisure activities.
Assist with Mobility and Equipment Use:
Help service users with mobility challenges and other physical disabilities, ensuring proper use of support aids and personal equipment to enhance their safety and comfort.
Ensure Health and Safety Compliance:
Adhere to all health and safety policies and procedures to maintain a safe environment for both service users and staff.
Engage in Professional Development:
Participate in regular one-to-ones with line managers, staff meetings, and training sessions, and take responsibility for developing your own knowledge and skills through continuous learning and experience.
Take on Additional Responsibilities:
As requested by management, take on additional duties following appropriate training and for personal development.
Perform Other Duties as Required:
Be flexible and willing to take on any other reasonable duties as required.
