About the organisation:

Operating for 50 years, Access Community Trust is a multi-award winning charity and provider of housing, youth services, health/wellbeing and employability services across Suffolk and Norfolk. Our ambition is to deliver a service driven by passion and enthusiasm that reaches all vulnerable and socially excluded/isolated people without exception.

Hours:

20 hours per week which will include a mix of day & evening working (on a rota basis)

Pay:

12.60 per hour

Purpose Statement:



Access Community Trust provides housing support to residents which is heavily focused on working them to provide personal resilience and skills to be able to live independently within the community. Each referred individual should be supported in a way which encourages residents to be empowered and to help build independence. A key part of gaining independence is being able to maintain an ability to live independently in the community and the role will focus on enabling our customer to build the skills and resilience to achieve this.

Principal Responsibilities:



The key responsibilities will require the postholder to work proactively and reactively with their caseload, ensuring regular keyworking sessions, risk assessment updates and connecting clients with internal and external support mechanisms which help to enrich the customer's journey and successful move-on.

Performance Indicators:



Key indicators are focused on the regular engagement with residents and their progress within their own individual journey up to and including move-on outcomes.

Experience:



We actively encourage those with lived experience to become support workers and therefore whilst vocational experience is heavily valued, so is that of someone with lived experience.

Full training can be provided to candidates evidencing the key skills listed above, however if you have previously worked in a similar role or have transferable skills then we would also welcome and encourage your application.

Key objectives and responsibilities of role

:

1.

To undertake proactive and reactive duties to ensure the project is secure and safe for the comfort and wellbeing of all residents and staff and/or visitors.

2.

To undertake suitability assessments in line with funding and organisational criteria.

3.

To effectively communicate via email, face to face, telephone and video conferencing and accurately record work undertaken.

4.

To help residents develop their independence through providing support, advice and guidance within a user-led framework and style. This will also include internal and external stakeholders and support providers.

5.

To proactively engage with internal and external stakeholders so as to check, monitor and report progress of interventions and ensure client progress is maintained and reviewed.

6.

To be able to provide reactive support when necessary to help the resident through difficult and negative times and provide encouragement on progress so that their journey to independent living is still a positive one.

7.

To work with customers in accordance with organisation procedures and processes including GDPR and safeguarding.

8.

To encourage and record client opinions, views and feedback to ensure customers feel empowered to drive a review and improve model.

9.

To keep up to date with all relevant information within the place of work. This will include resident focused and building focused information and developments as well as more generic information, for example Health and Safety

10.

To ensure residents keep to their basic commitments regarding (where applicable) payment, engagement, progress and respect to other residents and staff.

11.

To alert senior staff to issues regarding the resident such as non-payment, non-engagement and any other concerns such as safeguarding.

12.

To support the day to day running of the residence through undertaking tasks relating to clearing rooms and making them ready for new tenants.
