Duties
Assist individuals in planning and implementing personalised support programs that promote independence, community presence, and meaningful participation.
Collaborate with individuals, families, advocates, and professionals to facilitate effective communication, decision-making, and coordinated support services.
Support individuals in managing their personal finances, maintaining benefit entitlements, and adhering to tenancy agreements to ensure financial security and stable housing.
Identify and report risks, taking necessary precautions to ensure the health, safety, and well-being of individuals and visitors in line with housing partner and service agreements.
Participate in training and professional development to maintain and enhance specialist knowledge, ensuring high-quality service delivery.
Promote and maintain community inclusion and high-quality support provision at all times, ensuring individuals feel valued and respected.
Actively follow and implement company policies, procedures, and best practices, including those related to fire safety, health and safety, and equal opportunities.
Attend team meetings, supervision sessions, and appraisals to foster collaboration and maintain high standards of care.
Actively promote inclusion, equal opportunities, and anti-discriminatory practices in all aspects of service delivery.
Assist in identifying additional needs for individuals and providing tailored support to address them.
Perform other duties as required to meet the needs of the service, demonstrating flexibility and commitment to organisational goals.
Provide tailored support to individuals, ensuring their comfort, dignity, and independence.
Encourage and support individuals in developing life skills, such as budgeting, cooking, and managing their home.
Maintain accurate records of client progress, daily activities, and any changes in behaviour or health status.
Ensure a clean, safe, and supportive environment by adhering to health and safety protocols.
Skills
Experience or a strong interest in working with adults in a supported living setting, especially those with physical disabilities, sensory impairments, or mental health challenges.
Compassionate nature with a genuine desire to help others live independently and improve their quality of life.
Excellent communication skills to engage effectively with clients, families, and colleagues.
Strong organisational skills with the ability to manage tasks and maintain accurate documentation.
Proficiency in promoting independence and empowering individuals to make decisions about their lives.
Ability to remain calm and adapt to challenging situations while maintaining professionalism.
A valid driving license (preferred) to assist with appointments and community activities.
