Job Description

We are seeking a compassionate, resilient hardworking Support Workers to join a dedicated care team providing 2:1 support for a young man living with complex medical needs, including a tracheostomy and ventilator, alongside behaviours that can challenge. Supporting him in his own home alongside his family and helping him access leisure activities, attend college. This is a rewarding opportunity to make a real difference in the life of an individual requiring consistent, skilled, and person-centred care in his own home and community.

Key Responsibilities:



Provide 24/7 care, including tracheostomy care and ventilator management, following clinical protocols. Assist with personal care tasks, including showering, dressing, toileting, and mobility support Monitor and respond promptly to any signs of respiratory distress or equipment alarms. Support the young man with daily living activities, routine, and personal interests. Implement and follow positive behaviour support plans, working with other professionals as needed. Maintain accurate records of care provided and incidents, adhering to safeguarding and confidentiality standards. Facilitate social inclusion and community engagement in a safe and supportive manner. Communicate effectively with family members and the wider multidisciplinary team.

Requirements:



Essential:

Prior experience in care, SEN or likewise. tracheostomy and ventilator care training can be provided for the right candidate. Experience supporting individuals with challenging behaviour, ideally using PBS (Positive Behaviour Support) would be advantageous. Calm, patient, and confident under pressure. Ability to work as part of a small team and follow consistent routines and protocols. Excellent communication and observational skills. Physically fit and emotionally resilient. Willingness to work flexible shifts, including nights and weekends.
Desirable:

NVQ Level 2 or 3 in Health and Social Care (or equivalent). Valid driving licence (if transport is part of the role

Working Hours:



18-24 hours per week available

This will cover a variety of shifts including weekdays, evenings, weekends, and waking nights.

Weekends will be chill time, days out, taking our client to football matches and day care activities.

Additional hours may also be available.

Rates of Pay:



Weekdays: 17.00 per hour

Weekends: 19.00 per hour

Our client seeks someone who is well organised, flexible and can fit into a loving family environment.

A full clean driving licence is essential as you will be expected to drive our client's adapted vehicle.

All posts are subject to an Enhanced DBS Disclosure with Adult and Child Workforce Barring.
