We are seeking compassionate and dedicated Support Workers to provide high-quality care and assistance to individuals LOCATED IN THE WARRINGTON AREA WA1, WA3. including assisted living, home health.

Responsibilities
Assist residents with Activities of Daily Living (ADLs), such as bathing, dressing, grooming, and mobility support
Monitor patients' vital signs and observe for any changes in condition
Administer medications following care plans and HIPAA regulations
Support residents with memory care needs, including Alzheimer's and dementia care strategies
Prepare nutritious meals and assist with meal planning in accordance with dietary requirements
Utilize equipment such as Hoyer lifts and resident lifts to ensure safe transfers
Maintain accurate documentation of patient care activities and observations
Follow prescribed care plans and collaborate with healthcare professionals to ensure comprehensive support
Engage residents in social activities to promote mental and emotional well-being
Maintain a clean and safe environment through regular cleaning and infection control practices
Respond promptly to emergencies with appropriate first aid or CPR when necessary
Experience
Candidates should possess relevant experience or skills including:
Prior experience working in group homes, assisted living, nursing homes, or home & community care settings
Knowledge of patient observation, monitoring vital signs, and medication administration
Familiarity with Alzheimer's, dementia, autism spectrum disorder, or developmental disabilities care
Experience providing personal care (e.g., assisting with ADLs) and supporting residents' independence
Certifications such as First Aid and CPR are highly desirable
Background knowledge of HIPAA regulations ensures confidentiality and privacy compliance
Experience working with vulnerable populations requiring specialized support such as hospice or long-term care is advantageous
