MAIN PURPOSE:



To support the Therapy staff to provide high-quality holistic person-centred care and support to service users to reach their full rehabilitation potential.



MAIN TASKS AND RESPONSIBILITIES:



Together with the multi-disciplinary team responsible to ensure optimum cognitive, social, emotional, physical and functional needs are addressed. To support service users with cancer or other life-limiting illnesses, who have specialist palliative or neurological needs Responsible to ensure that the service users' individual rights and choices are actively promoted and supported, and their beliefs and preferences appropriately acknowledged Responsible to report status and variances in treatment and condition to the appropriate Therapist at the earliest opportunity and progress treatment as advised by the Therapist Where appropriate, responsible to review, order and fit equipment and work with service users and their carers/family to demonstrate its safe use, either in the centre or in service user's home Responsible to liaise with service users and family members to help complete forms, gather information and co-ordinate interventions To support the Therapy Team by completing administrative tasks such as gathering referrals, recording monthly activity data Responsible to collect, collate, evaluate and report information, maintaining accurate service user records in all formats, including written and electronic information systems Facilitating the safe and smooth running of the Service, the post holder is expected to maintain a safe working environment, maintain service user confidentiality, and occasionally taking on reasonable additional tasks

ABOUT YOU:



Educated to GCSE level or QCF Level II or equivalent in recognised health and social care qualifications. Understanding of health/social care sector work Experience of working with people in a care setting is desirable but not essential Skilled in interacting with all types of people Proven experience of working within a team Good communication skills, both verbal and written Able to work within policies and procedures An ability to work under supervision but also autonomously when appropriate An ability to prioritise, organise and coordinate daily workload An ability to build effective working relationships

Purpose

This role carries no line management responsibilities The post holder will be expected to work effectively with the Therapy team, the wider Multi-Disciplinary Team(s) and the community Responsible for working alongside and/or line managing volunteers within their team

Qualifications required

No formal qualifications required Evidence of an appropriate level of literacy and numeracy

Complexity and Accountability

This post holds no budgetary responsibility but will be expected to contribute in helping to manage costs where appropriate The post holder is expected to be mindful of property and resources, and safeguarding personal possessions Providing and receiving routine information To communicate effectively and sensitively with service users and their families/friends/carers in situations that may be difficult and emotionally distressing Deliver care according to Brainkind policies and procedures
