Job Summary:
In a clinical general practice (GP) setting, a triager plays a critical role in managing patient econsultation flow and ensuring that medical care is prioritized effectively. Our team play a vital role in channelling patient requests and ensuring that every individual gets timely and appropriate care. You will work closely with our dedicated clinicians to oversee the daily influx of patient econsultation requests. Your role is pivotal in ensuring that patients receive the right care, at the right time, as directed by the triaging clinical team.
Here are the key responsibilities and tasks typically associated with the role of a triager in a GP setting:
Key Responsibilities:
Clinical & Administrative Triage:
Direct clinical inquiries to the Triage Clinician, other Allied Health Professionals (AHPs), and administrative queries to the appropriate team member.
Inter-Team Collaboration
: Collaborate closely with in-house Pharmacists, Social Prescribers, First Contact Practitioners, GPs, and nurses, ensuring that patients are guided to the appropriate professional.
Service Development:
Partner with colleagues & your line manager to enhance the online triage model and contribute to its evolution through evaluation, audits, and system research.
Record Keeping
: Maintain meticulous, up-to-date records of all patient interactions using the available software systems.
Communication
: Communicate effectively with patients, providing clear instructions and information about their care. Liaise with doctors, nurses, and other healthcare professionals to convey important information about patient status and needs.
Practice Support:
Assist the practice in meeting Quality and Outcomes Framework (QOF) targets, Key Performance Indicators (KPIs), and other locally commissioned enhanced services.
Medication Records
: Accurately record patient information, including symptoms, medical history, and triage decisions. Maintain detailed records to ensure continuity of care and facilitate follow-up treatments.
Coordination
: Coordinate patient appointments and manage the schedule to optimize the clinic's workflow. Direct patients to appropriate resources, such as specialist services or emergency care, as requested
