x1 position for Saturday working (5 hours a week)



AfC Band 6 per annum incl. HCAS (pro rata) PLUS weekend enhancement

The West Middlesex Site has a vacancy for pharmacists to work regularly in dispensary at weekends/public holidays. This role includes provision of legally required supervision of staff as well as fulfilling professional duties.



We currently require a pharmacist for Saturday .



You will work as part of a team consisting of Pharmacists, Technicians and Assistants. The work primarily consists of the clinically screening, dispensing and checking prescriptions for inpatients and discharge medication.



You will be required to attend the department during weekday hours for induction and training.



Further weekday/weekend hours may be available depending on service needs on staff bank.

1. Dispensary



Provision of legally required supervision and validation aspects of dispensing processes.

Dispensing and clinically screening medication for in-patients (including controlled drugs), checking technician/assistant/pre-registration pharmacist dispensing according to local, national and legal standards. Pharmacists help manage stock control, dispensing workload, dispensary staff and contribute to the maintenance of a quality service that relates to clinical governance issues.



2. Clinical Pharmacy Services



Clinical screening of prescriptions for ward inpatients, discharge medication and outpatient prescriptions from AEC and A&E.



Identification of pharmaceutical problems of patients and application of therapeutic knowledge gained from clinical training programmes and experience as a practising pharmacist. This includes:



Ensuring that inpatient prescriptions are:

safe, accurate, legal, clinically appropriate for the

individual patient, cost effective and compliant with local prescribing policies. Recommending

changes to the prescription to ensure the above.



Advising other professional staff on medicines management issues



Participating in clinical intervention systems and other quality monitoring initiatives



Implementing local formulary recommendations/therapeutic guidelines and other related support to Divisional/Specialist Pharmacists



Implementing risk management and other Trust medicines related policies



Attend relevant training required for the role (e.g. Cerner training, JAC training, Trust induction, mandatory training modules)



Effective use of Cerner system to screen ward orders and TTAs.



Prioritisation and resolution of pharmaceutical problems involving all other healthcare professionals when necessary



Responsibility for the supply of prescribed medication to patients on wards in a timely manner.



Undertaking training in specialist areas, such as HIV, anticoagulation services, cardiology, surgery, pre-assessment clinics, etc where required.



Adhere to the departments 'weekend working guide' to follow relevant processes

Deputising for relevant senior staff



Participation in performance review and appraisal and identification of own learning and development needs in order to meet Knowledge and Skills Framework profile



Assisting in training and supervision of pharmacy technicians/assistants and pre-registration pharmacists



Maintaining a CPD portfolio and meet all relevant CPD requirements of the General Pharmaceutical Council



Promotion of safe, economic and effective use of medicines



Undertaking any other duties as agreed with the Chief/Deputy Chief Pharmacist.



COMMUNICATION



Ensure all communication, which may be complex, contentious or sensitive, is undertaken in a responsive and inclusive manner, focusing on improvement and ways to move forward.



Ensure all communication is presented appropriately to the different recipients, according to levels of understanding, type of communication being imparted and possible barriers such as language, culture, understanding or physical or mental health conditions.
