Context & Purpose:



The Lantern are working in partnership with Dorset Healthcare, BCHA and Help and Care, to deliver wellbeing hubs, across Dorset.. This is part of the universal offer of wellbeing support, an element of the mental health transformation programme for adults. The hubs will support people to take the steps that matter to them, by connecting them to what they might need, at the right time and in the right place, ultimately empowering people to make their own choices and moving towards full citizenship within their communities. Citizenship in this context means having a voice, a sense of belonging, purpose, identity and equal access to the rights and opportunities that should be available to all.

Key Responsibilities & Duties



You will Create a warm and welcoming first point of contact at the hub, building rapport and relational safety to create a space where people feel safe and welcome.

To consider the person as the expert on themselves, employing a strengths-based approach, acknowledging the strengths and skills an individual brings, supporting self-management while recognising that capacity for this may change over time

Enabling and supporting choice, making decisions collaboratively with the person and being guided by their preferences. Advocating for the person at multi-disciplinary/interface team meetings and collaborative triage, knowing when to escalate concerns

Promoting the range of local resources, groups and opportunities to encourage positive wellbeing experiences within the community and to support warm transfers to relevant service encouraging the person to advocate for themselves and act as independently as possible

To develop an 'Individual Learning Plan' for the person to access Recovery Education courses through the Recovery Education Centre

To co-produce a 'My Wellbeing Plan' (or an alternative if individual prefers) collaboratively with individuals, agreeing goals that are important to them

To co-create safety plans, with a focus on safety rather than risk, which support the individual to utilise their personal, community and service resources to maintain their safety

To provide support through face-to-face conversations, virtual conversations, phone-based conversations, on a one-to-one basis as well as in group settings

To use a networked approach to support (using the principles of Open Dialogue- training will be provided) to support communication with a person's network, and facilitate support to families, carers, and others as appropriate and while respecting the person's preferences around confidentiality

To be able to effectively communicate any care planning, risk or safety or safeguarding concerns to colleagues in particular when participating in the virtual MDT

To develop an understanding and response to health inequalities of communities which the hub serves. To have effective strategies to engage with people who have been underserved by health and social care

Support and enable local health, social care, and voluntary sector professionals to make appropriate referrals to the service, through building relationships, networking, and partnerships

Accurately record and collect relevant information on record systems, seeking consent and collaboration from the person where possible

To effectively manage caseloads and appointment scheduling, with consideration to modelling self-care and wellbeing

To promote active citizenship, focusing on what a person has to offer (note communities are not just geographic but can also be communities of identity), encouraging them to participate and contribute both within their communities and to the service

To engage with local communities and community groups to build relationships and build understanding of how to provide a supportive environment for people with mental health challenges

To commit to ongoing reflective practice and supervision

To support service users with significant emotional distress and be exposed to hearing about distressing situations

To engage in ongoing learning and development at an individual and at service level

To maintain professional conduct with clients and professionals at all times, modelling healthy boundaries

Be able and aware of social and wider determinants of health and models such as health and digital literacy and supporting behaviour change

o understand the value co-production and lived experience and be able to effectively work alongside clinicians, colleagues in the VCSE sector and those intentionally employed to use their lived experience

To ensure regular review of risks and issues that could impact on delivery

To work within the values, ethos, and vision of employing organisation

To contribute to the service's marketing, promotion, and publicity

To work in accordance with the policies and procedures and all relevant legislation

To undertake any other duties as required, appropriate to the post

Skills and Knowledge Required



Listening, communication and relationship building skills tailored to different individuals with awareness of potential barriers Ability to support people to identify their own goals and sharing information and resources to enable them to achieve those Ability to model healthy boundaries and communicate these with transparency and kindness Understanding of safeguarding and risk assessments Ability to adapt to new ways of working and support service development An awareness of the wider non-clinical issues that are important for people Strong working knowledge or ability to learn about local organisations and resources (including voluntary and community sector organisations, NHS, social care) Proficiency in MS office, and ability to record accurately using a range of electronic systems for both client and external network contacts

Desirable Skills



Group facilitation skills Skilled in case management Experience of collecting relevant information and recording on relevant systems contributing to the evaluation of a service. Ability to use feedback from people using the service to consider any development opportunities for service improvement

Qualifications



5 GCSEs or equivalent work experience Willingness to complete any study and training as necessary and appropriate to the role Motivational Interviewing and/or Brief Solution Focused Therapy or similar (desirable) Training in Open Dialogue (desirable)

Experience



Experience of utilising listening skills and working in a person-centred way. Experience of working in community or support-based roles, in a paid or voluntary capacity Experience of working under own direction, managing workload. Experience of using a trauma-informed approach

Meets DBS reference standards and criminal record checks



Ability to work flexible hours and travel out of area as necessary



This is a Fixed Term role expected to end in March 2029
