Job Description

We are looking for a motivated individual to work within the Integrated Discharge Team at Watford General Hospital.



The Discharge Coordinator is a Band 4 position full time hours at 37.5 PW. This is a secondment for 10 months. This also includes covering a 7 day service.

The Discharge Coordinator will act as the point of contact for the ward teams, AHP's, IDT and SPOC promoting an effective and smooth discharge for all patients on their designated wards.



They will be responsible for highlighting any referrals which the DPN will need to complete. They will then ensure that all aspects of the patient's discharge is progressed and escalate when there are changes or delays to the EDD, patient needs or clinical condition.



With a new hospital planned for Watford and work underway to update theatres at St Albans City Hospital, this is an exciting time to join us.

A key part of the role will be to ensure there is early communication with families/carers/care homes to ensure all relevant information is collated and potential barriers are highlighted so they can be addressed and solved prior to when the patient leaves hospital and mitigate failed discharge.



Alongside these elements the post holder will be responsible for ensuring accurate data is captured on all data bases in order to fulfil all Trust reporting requirements.
