About the Post:





An exciting opportunity has arose to join our Brokerage Team as a Brokerage Officer.


As a Brokerage Officer you will effectively broker support packages on behalf of individuals assessed as having an eligible need for support. The postholder will be responsible for liaising with a range of care providers to secure appropriate packages of care, and will organise innovative, value for money services, co-ordinating appropriate services to meet desired outcomes, which promote strengths-based practice. The postholder will also be responsible for accurately recording care provisions on our case management system.



You will accurately provision services on the Adult Social Care system in a timely manner, ensuring records reflect the type and cost of support and include any relevant income markers to support the councils financial planning and ensure care providers are paid for services delivered. Referrals will include support for home care, residential / Nursing care, respite provision, specialist residential accommodation, Supported Living, Shared Lives and any other appropriate services.



You will operate in accordance with all statutory frameworks and Care Acts, Caldicott Principles, relevant social care legislation and procedures as well as council policies, procedures and frameworks.
