Care Coordinator Location: Shah Zaman Surgery Hours: 12 hours per week across 3 days Contract: Employed under the NHS DES (Additional Roles Reimbursement Scheme) Accountable to: Practice Manager / Safeguarding Lead Base: Shah Zaman SurgeryJob Summary The Care Coordinator will support the delivery of personalised, proactive, and coordinated care for patients, with a particular focus on safeguarding and patient recall processes. This non-clinical role is central to improving patient outcomes by ensuring individuals receive the right care, at the right time, in the right place. The post holder will work closely with the practice team, social prescribing link workers, and other professionals to support patientsespecially those with complex needs or requiring safeguarding interventions. They will also assist the administrative team with incoming calls and general practice support, contributing to the smooth running of day-to-day operations.Key ResponsibilitiesPersonalised Care Coordination Identify and manage a caseload of patients requiring proactive support, including those with long-term conditions, frailty, or complex needs.



Develop and maintain personalised care and support plans in line with national guidance and what matters to the patient. Facilitate shared decision-making conversations and support patients in understanding their care options. Help patients access services, community resources, and self-management tools to improve their health and wellbeing.Safeguarding Coordinate and arrange safeguarding meetings in collaboration with the Safeguarding Lead. Take accurate and confidential minutes during safeguarding meetings.



Maintain and update the practice safeguarding register. Escalate safeguarding concerns appropriately and ensure timely follow-up. Liaise with external agencies and professionals involved in safeguarding cases.Patient Recalls Manage and coordinate patient recall systems to ensure timely reviews and follow-ups. Work with clinical staff to identify patients due for chronic disease reviews, immunisations, or other routine care.



Contact patients, book appointments, and ensure accurate documentation of recall activity.Administrative Support Assist the administrative team with answering incoming telephone calls and responding to patient queries. Provide general administrative support as needed to ensure the smooth running of the practice. Support reception and back-office functions during busy periods or staff absence.Communication and Navigation Act as a point of contact for patients and carers, helping them navigate health and care services. Provide high-quality written and verbal information to support patient understanding and decision-making.



Liaise with internal and external services to ensure continuity of care.Multidisciplinary Team (MDT) Support Support the coordination and delivery of MDT meetings. Contribute to case discussions and ensure actions are followed up. Work collaboratively with GPs, nurses, social prescribing link workers, and other professionals.Training and Development Undertake or be enrolled in appropriate training as outlined by the Personalised Care Institute and the Workforce Development Framework for Care Coordinators. Participate in supervision and ongoing professional development.



Maintain up-to-date knowledge of safeguarding procedures and personalised care approaches.
