We are seeking a dedicated and compassionate Care Coordinator to join our team. In this vital role, you will oversee and coordinate tailored care plans for our clients, ensuring they receive high-quality, consistent support that promotes their wellbeing and independence.
Responsibilities
To collaborate with our leadership team.
To assess client needs and develop personalised care plans and risk assessments.
Coordinate and schedule care reviews and visits.
Monitor and review client progress and adjust care plans as needed.
Act as a point of contact for care staff, clients and their families - managing the reporting platform, family portal and upholding open lines of communication.
Ensure compliance with regulatory standards and company policies.
Lead and support care staff, providing guidance and training in reporting pathways.
Maintain accurate documentation and care records, carry out audits and reports in line with company standards.
Job Types: Full-time, Part-time
Pay: From 12.25 per hour
Expected hours: 16 - 36 per week
