Position Summary:


The Care Coordinator plays a pivotal role in facilitating and coordinating comprehensive care services for individuals within healthcare or social service settings. They are responsible for assessing the needs of clients, developing care plans, coordinating with various healthcare providers, and ensuring the delivery of high-quality, person-centred care. The Care Coordinator serves as a liaison between clients, families, caregivers, and healthcare professionals to optimise health outcomes and promote overall well-being.

Key Responsibilities:



Client Assessment: Conduct thorough assessments of clients' physical, emotional, and social needs to determine appropriate care interventions. Care Planning: Develop individualised care plans in collaboration with clients, families, and interdisciplinary teams, outlining goals, interventions, and timelines. Coordination of Services: Coordinate and facilitate access to healthcare services, including medical appointments, diagnostic tests, therapies, medications, and community resources. Communication: Serve as a primary point of contact for clients and their support networks, providing ongoing communication, education, and support regarding care plans and available resources. Advocacy: Advocate for clients' rights and preferences within the healthcare system, ensuring their voices are heard and their needs are met. Documentation: Maintain accurate and up-to-date documentation of client assessments, care plans, progress notes, and coordination efforts in compliance with regulatory standards and organisational policies. Collaboration: Collaborate with healthcare providers, social workers, case managers, and other professionals to coordinate seamless transitions of care and promote holistic care delivery. Crisis Management: Respond promptly to crisis situations, assess risk factors, and implement appropriate interventions to ensure the safety and well-being of clients. Quality Improvement: Participate in quality improvement initiatives aimed at enhancing the effectiveness and efficiency of care coordination processes. Professional Development: Stay abreast of current trends, best practices, and regulatory requirements in care coordination and related fields through ongoing education and professional development activities. Additional duties: Manage household expenses and inventory, support care workers, manage training schedules, and ensure well-being everyone within the supported accommodation.

Qualifications:



Health and Social Care Level 2/3 or equivalent Understanding of the CQC Framework Knowledge of Health and Safety within the Care Sector Experience of undertaking Risk Assessments Relevant experience in care coordination, case management, or healthcare administration. Excellent IT skills (word, Excel, PowerPoint) Strong understanding of healthcare systems and regulatory requirements. Excellent interpersonal, communication, and problem-solving skills. Ability to work collaboratively in interdisciplinary teams and independently manage multiple priorities. Proficiency in documentation and electronic health record systems. Commitment to cultural competence, diversity, and inclusion in healthcare delivery. Experience of using Care Control software (desirable)
