Overview


The Care Team Leader plays a vital role in providing high-quality care and support to individuals in various settings. This position requires a compassionate and dedicated individual who can lead a team of caregivers while ensuring the well-being and safety of service users. The Care Team Leader will be responsible for overseeing daily operations, implementing care plans, and fostering a supportive environment for both staff and Service users.

Responsibilities



Supervise and support caregiving staff in delivering exceptional patient care. Develop and implement individualized care plans tailored to each resident's needs. On Call Duties on alternate weekends Must be flexible and reliable due to nature of the job Monitor Service users' health status, including vital signs and medication administration. Assist Service users with Activities of Daily Living (ADLs), such as bathing, dressing, and meal preparation. Ensure compliance with HIPAA regulations and maintain confidentiality of patient information. Provide training and guidance to staff on best practices in caregiving, including first aid and CPR techniques. Facilitate communication between Service users, families, and healthcare professionals regarding care needs. Maintain a clean and safe environment for Service users doing spot checks and observations on care staff. Participate in ongoing assessments of Service users' conditions, including memory care for those with Alzheimer's or dementia.

Skills



First aid certification and CPR training are essential. Level 3 or working towards. Strong patient observation skills to monitor changes in health status. Knowledge of cleaning protocols to maintain hygiene standards. Familiarity with home & community care practices is beneficial. Experience assisting with ADLs and providing patient care is required. Cooking skills for meal preparation tailored to dietary needs are advantageous. Background in nursing home or long-term care experience is desirable. Understanding of memory care principles for individuals with dementia or Alzheimer's disease is a plus. Knowledge of developing care plans that meet individual resident needs is important. Strong interpersonal skills for effective communication with residents, families, and team members. Job Type: Full-time

Pay: 13.00-18.60 per hour

Expected hours: No less than 40 per week
