Job Title:

Clinical Lead

Reporting to:

The St George's Nursing Home Registered Manager/Directors

Working hours: Total 30 hours; split into

(circa 12 hours/week - lead role, circa 18 hours of registered nursing role).
To work weekly contracted hours, to be mutually agreed with the nursing home manager/directors based on the needs of the Home.)

Job Purpose:



The lead will have overall clinical responsibility and accountability for the clinical care at St Georges nursing home. He/she will oversee the work of nursing and care team members.

He/she will ensure the home's policies and procedures are adhered to, in accordance with the highest care standard and guidance.

Job Duties:



Management:



Demonstrate - clinical care and managerial leadership to RGN, senior, carers and ancillary staff, as appropriate, allowing opportunities for staff to communicate openly and positively. Provide a visible, accessible and authoritative presence within the clinical/care areas of responsibility for residents, relatives and staff, providing help, advice and support at all times. Support, supervise and advise nursing team leaders to ensure that care delivery meets all residents' identified needs. Organise, formulate agendas for and hold regular staff meetings, in line with St George's nursing home's meetings policy. Assist the nursing home manager with staff training, supervisions and appraisals. Ensure that all new staff receive a comprehensive induction programme and help the home manager to support them during their induction. Be aware of staff educational needs, communicating them to the home manager. Ensure that these are met, so that all staff have appropriate skills and knowledge to deliver competent and timely resident care. Pass on skills and knowledge to colleagues via supervision and mentoring, in line with their own individual competencies.

Clinical:



Have a good knowledge of Carebeans (Care plans) - to be the Carebeans champion of the home. Have sound knowledge of safeguarding, DOLS and the Mental Capacity Act and put this into practice, guiding others and seeking support, as required. Ensure that the home's clinical/care performance meets required fundamental standards. Ensure the safe delivery of nursing care, maximising nursing and other resources. To develop clinical pathways in the nursing home to promote efficiency - such as using electronic systems and minimise use of paper. Take responsibility and lead the electronic prescribing safe ordering, administration and safekeeping of drugs and treatments, as prescribed and in accordance with the home's policies, if applicable, and NMC Guidelines. Take responsibility for auditing care delivery, developing action plans to rectify areas of weakness, monitor and report to the home manager/directors. Organise time effectively and act autonomously to assess, monitor and identify clinical care needs, ensuring the appropriate use of equipment to maximise potential effectiveness. Oversee residents' activities and cooperate with Activities Manager for the Residents based on what they would usually perform (or have in the past performed) themselves but cannot now do so without assistance. Encourage and support the optimum level of independence while maintaining residents' dignity and self-esteem. Help the nursing home manager with pre-admission assessments and arrangements that are necessary for admissions to the home. Regularly evaluate and review the effectiveness of care plans, implementing changes in care when outcomes do not meet care plan goals.

General

:

Be aware of and understand St George's nursing home's philosophy of care, contributing to the on-going development and review. Be aware of the need for budgetary control. Be aware of the administrative arrangements for the day-to-day running of the home. Ensure that staff understand and comply with the health and safety policy. Achieve and promote all relevant standards and codes of conduct including NMC and CQC core standards. Keep the manager informed of any issues/potential issues regarding communication with residents and families, members of the home's multi-disciplinary team and other providers of care and services. Pre-empt staffing issues, changes in residents' health, well-being or social needs, potential safeguarding issues, concerns relating to the home environment or any staff/resident incidents or accidents within the home. Develop and maintain your own competence in basic and advanced clinical/care skills. Identify and act on development needs for nurses and carers To follow the home's written policies and procedures. Provide on-call and emergency cover in a pattern to be agreed with the nursing home manager. This may also include night shifts, as and when required. Complete any mandatory training to ensure competency to fulfil the responsibilities of the role.

Specific Qualifications/Skills/Attributes:



To be the nursing home - Carebean champion Rota management - ensure rota is up to date with Deputy and assist in last minute cancellations. This job description is not exhaustive and duties may vary according to the needs of the home which will be discussed with the Clinical Lead.
