As a Community Enabler, you will provide person-centred support to individuals within their own homes and local communities. Your role is to enable people to live as independently as possible, promoting well-being, inclusion, and active participation in daily life. You will assist with practical tasks, social activities, and personal care (if required), helping individuals maintain control over their lives and connect with others.
Duties
Support individuals to engage in their local community and participate in activities that promote social inclusion and independence.
Assist with everyday tasks such as shopping, attending appointments, and taking part in leisure activities.
Provide companionship and emotional support to reduce loneliness and isolation.
Where necessary, assist with personal care in a respectful and dignified manner.
Encourage and support individuals to set and achieve personal goals.
Promote choice, control, and independence in all areas of life.
Work in a person-centred, respectful, and empowering way at all times.
Keep accurate records and report any concerns regarding health or safety.
Liaise with families, health professionals, and community organisations when needed.

Desirable:
Previous experience in a care or support role.
Understanding of community-based support and social inclusion.
NVQ Level 2 or 3 in Health and Social Care (or willingness to work towards one).
Driver's license and access to a vehicle.
Excellent interpersonal skills with the ability to build rapport with clients and their families.
