Community Social Worker - NPT Hospital

Contract Type:

Temporary - 6 Months with potential for further extension.

Hours:

Full Time 37 hours per week

Location of post:

Neath Port Talbot Hospital

This position includes a market pay supplement.

For a

Grade 9 Senior Social Worker

, the full-time equivalent salary is

42,298 to 46,478

, which includes a

3,000 supplement.*
For a

Grade 8 Social Worker

, including newly qualified staff, the full-time equivalent salary is

37,124to 40,513

, which includes a

1,000 supplement.*

This supplement is subject to review every 12 months.

An exciting opportunity has arisen to join a dedicated and enthusiastic team that supports individuals preparing for discharge from hospital.


Our team is committed to delivering high standards of care within a positive and collaborative working environment. We work closely with individuals to understand what matters most to them, carrying out Mental Capacity Assessments, Continuing Health Care Assessments, and leading Best Interest Meetings to ensure safe and timely hospital discharges.

Ideally you will have;



Applicants will need to be social work qualified and professionally registered as a social worker with Social Care Wales.

You will need a knowledge and working practice of the Social Services and Wellbeing (Wales) Act 2014, to include outcome focussed assessment and care planning, knowledge and working practice of Continuing Health Care, the Mental Capacity Act 2005 including Deprivation of Liberty Safeguards.
