We are looking for a Practice Nurse who has a special interest in Diabetes to join our friendly and proactive dispensing practice for 10 hours a week.
The successful candidate will deliver safe and effective nursing care to our patients and working in accordance with the NMC. The post holder will provide a holistic approach to patient centred care and with a particular interest and / or experience in managing patients with Diabetes.
You will be responsible for reviewing their treatment, blood test results and providing education and health management to optimise their health and wellbeing.
We have an excellent, dedicated and loyal team working from purpose-built premises with a list size of approximately 6000 patients. We use TPP SystmOne. Working with 7 GPs, 3 PN's and 3 HCA's.
