Purpose of the Job:




To work as part of the Reach team alongside the Community Mental Health Teams (CHMTs) to support clients who have a dual diagnosis of serious mental illness (SMI) and substance / alcohol use. You will support people to reach their goals and move towards a meaningful recovery personal to them.


Main Duties:




Hold a caseload of clients, providing one to one support for individuals in a recovery-focused and person-centred way, which builds confidence and helps people to move forward. Use your own lived experience of recovery from mental health problems, as appropriate, to inspire and support others. Ensure that client notes are recorded in an accurate and timely manner and kept updated. Work in line with all relevant safeguarding policies and procedures, ensuring concerns are escalated appropriately. Work closely with other health and social care professionals including statutory and voluntary agencies to ensure that support is joined up and of high quality. Deliver support through carrying out weekly face to face appointments with clients, via home visits, the community, activities, community mental health teams, other meetings or appointments with third party professionals.

Qualifications to GCSE Level 4 or above, or equivalent experience
