Summary of role



This role is hybrid and your work will be split between the Maria Care main office and working out in the field to support carer training and quality checks. Responsible for implementing, monitoring, and maintaining field based quality control systems and supporting all Community carers.

Principal responsibilities



To carry out Individual Needs Assessments for new and potential service users.

To conduct risk assessments as part of the Individual Needs Assessment and update these as required as part of the individual's risk management.

To carry out introductory visits for new service users where required.

To complete Initial visits following Hospital Discharges to ensure medication is set up correctly.

To support the Care Coordinator in ensuring all service user visits are always covered.

To be part of the on-call team in case of emergency.

To support the office team with administration and day to day functions.

To liaise closely with the Care Coordinator regarding recruitment, permanent allocation of community carer's and the writing of Care & Support Agreements.

To monitor EMAR on a weekly basis and work with management to investigate errors, support and retrain, and competency check new staff where appropriate.

To identify suitable community carers to cover for any absences, which may be due to sickness or holiday.

To allocate suitable alternative community carer's when an existing community carer notifies that they are unable to attend a call.

To facilitate the practical induction of all new community carers.

To provide and facilitate in-field training as and when appropriate.

To identify any additional training needs of community carers and report accordingly.

To conduct service user reviews and respond to Individual Review Forms completed by community carers as soon as possible.

To urgently report any changes in a community carer's or service user's schedule to the Care Coordinator.

To support, monitor, supervise and mentor all community carer's in the field.

To provide support to all community carer's in the field.

To monitor all service users for service and community carer's satisfaction.

To ensure accuracy and completeness of all documentation held in service users' homes.

To encourage and identify opportunities for community carer's continuing professional development.

To ensure that all written reports and administration is up to date.

To maintain close liaison with both the Registered Manager and Care Coordinator.

In emergency situations, complete care calls to support our carers in the community.

Field Care Supervisors are responsible for the efficient running of their own specific, defined areas and should vary their hours of work in order to monitor and support all community carers and service users effectively. Field Care Supervisors should spend approximately 50% of their working week in the field with the remaining 50% being spent in the office in order to complete the necessary administrative duties.

Working closely with the Registered Manager and Care Coordinator you will be a key part of compliance and operations in the day to day and long term planning of Maria Care. supporting each other as a team and encouraging those around you will be key parts of your job.

Qualifications / Experience



Proven management and supervisory experience in a care setting Strong knowledge of patient care and anatomy Experience in senior care Familiarity with dementia and Alzheimer's care Background in team management and case management Previous experience in home care or nursing care settings Ability to provide compassionate care to Adults Strong IT skills

Pay: From 28,275.00 per year

Expected hours: 40 per week
