You will be working as a Frailty Co-ordinator for Highcliffe Medical Centre. This role is pivotal to ensuring that all patients receive the best possible care and service.

You will support the Frailty team with improving access to services, providing advice, and information, and ensuring health and care planning is timely, efficient, and patient-centred. The role will include supporting digital initiatives and includes responsibilities for the co-ordination of the patients journey through primary care.

Key Responsibilities:



Work within the Frailty Team and alongside other primary care professionals to support our frail and elderly patients. Serve as the initial point of contact for the frailty team. Filter tasks to determine if a visit is necessary, prioritise tasks, and assign appropriate staff based on their capacity and qualifications. Follow HOT script guidelines for booking visits. Gather detailed information when booking visits, including patient relatives, next of kin, consent, mobility, and any specific concerns or queries. Ensure joint visits are arranged where necessary. Manage a waiting list for patients over 75 recently discharged from the hospital. Follow up with patients within a week of discharge to ensure they are coping. Code follow-ups appropriately and handle delayed discharge summaries promptly. Determine if further follow-up is needed based on patient interactions with GPs or clinic visits. Code and note patients discharged to care homes or respite care. Direct patients to appropriate external services, such as Christchurch Angels, CHS Healthcare, BCP Council, Adult Social Services and Social Prescribers. Collaborate with team members to spread QOF-related visits throughout the year. Focus on QOF categories relevant to the frailty team, such as Dementia, Diabetes, Asthma/COPD, Heart Failure, and Learning Disabilities. Run reports for housebound and nursing/care home patients eligible for Covid/flu vaccinations. Maintain up to date PGD documentation, including printing, filing, and updating content pages. Process acute medication requests. Code unpaid carers and provide support, including distributing care packs and signposting to relevant resources. Stay updated with carers' meetings and newsletters. Document and save minutes for Frailty and Nurse meetings, sharing with relevant team members.
Job Types: Full-time, Part-time

Pay: 13.50 per hour

Expected hours: 30 - 37.5 per week

Schedule:

Monday to Friday No weekends
