General Practice Assistant (THG PCN)

Job summary



Trinity Health Group PCN are looking to GP Assistant at 24 hours a week to work as part of the multi-disciplinary team within primary care, supporting the smooth running of the surgery and the outreach team. Roles tend to focus primarily on a combination of administrative support and routine clinical duties.

GP Assistants are required to either hold prior to appointment the GP Assistant Certificate, or to undertake the Certificate if they do not already have the qualification.

Main duties of the job



Key responsibilities include arranging appointments, referrals and tests, and monitoring and drafting clinical correspondence. Those involved in clinical duties are likely to perform a range of routine procedures under direction including, but not limited to phlebotomy, blood pressure, pulse checks, ECG, BMI, health checks, LTC Part 1, assisting minor surgery, joint injections.

Trinity Health Group is a PCN made up of four practices on the west of the district (Trinity Medical Centre, Maybush Medical Centre, Crofton and Sharleston Medical Practice and Warrengate Medical Centre). The PCN team currently consists of Clinical Pharmacists, Pharmacy Technicians, Paramedics, Mental Health Practitioners, FCPs, Care Coordinators, Health & Wellbeing Coaches, GPAs and Social Prescribing Link Workers. Job responsibilities



Arrange referrals on behalf of the clinician internally to other clinicians or care coordinators/social prescribers/health and wellbeing coaches and externally to hospital or community clinics; Arrange appointments, referrals, tests, and follow-up appointments for patients; Support clinicians with administrative tasks, input to the clinical computer system and coding; Complete basic (non-opinion) forms and core elements of some forms for the GP to approve and sign (such as insurance forms, mortgage, benefits agency forms); Under the direction of the Clinical Supervisor, liaise with external agencies, such as arranging admissions; Sort clinical post and prioritise for the GP, referring to the appropriate person; Extract information from clinical letters that needs coding and adding this to patient notes; Support the recall system for Long Term Condition reviews; Those with clinical responsibilities will be responsible for preparing patients prior to their appointment, taking a brief history and basic clinical observations in readiness for the appointment; Perform a range of routine procedures under direction including, but not limited to Phlebotomy, blood pressure, pulse checks, ECG, BMI, health checks, LTC Part 1, assisting minor surgery, joint injections; Support the Practice with infection control activities; Responsible for the management of drug stock, including vaccines and injectables. Ensure adequate stock in clinical rooms; Complete simple clinical observations/investigations and processes as directed by the Clinical Supervisor, such as dipstick urine, taking blood pressure, ECGs and phlebotomy, following Practice protocol; Support with QOF reviews; Develop and maintain effective and positive relationships with multidisciplinary professionals, practice staff and other teams, facilitating a co-ordinated approach, liaising with them in the best interest of the patients care; Update patient documentation and computer systems as required in an accurate and timely manner; Provide patient focussed care, often acting as first point of contact for care needs; Develop excellent relationships with patients and carers in order to establish trust and encourage communication which puts the patient and their needs at the centre; Work within the parameters of own skills and knowledge, recognising when it is necessary to bring in other expertise or ask for further guidance from peers or supervisors; Work effectively as part of a multi-professional team in collaboration with other members of the service; Maintain an awareness of developments in practice; Ensure patient confidentiality at all times; Assist with the training and development of others where required; Follow all Company, PCN and Practice Policies and Procedures including those relating to confidentiality, health and safety; Attend Company, PCN, team and Practice meetings and updates as required; Access relevant networks and partnerships to share good practice, expertise and resources; Any other duties and responsibilities in keeping with the level and nature of the role.

Qualifications



Essential



GCSE English and Mathematics grade C or grade 4 and above or equivalent Literacy and Numeracy skills level 2 Willingness to undertake relevant training and GP Assistant Certificate
