About the Role:

Surbiton PCN LTD are looking for a proactive and confident part-time GP Assistant to join our friendly team. The successful candidate will work at Brunswick Surgery for 12 hours per week (Surbiton Health Centre) as part of a job share and will play a key role in supporting the management of patients with long-term conditions, including hypertension, diabetes, and other chronic illnesses.

Brunswick is an established training practice for VTS/medical students. The practice is increasing it's list size, and currently has 9,100 registered patients. The current GP clinical team consists of 4 Partners, 3 Part-time Salaried GPs and 1 Trainee. They are supported by a friendly multi-disciplinary and non-clinical team. Brunswick utilise EMIS Web clinical system and have an excellent QOF achievement.

Key Responsibilities:

Assist in the coordination and management of patients with long-term conditions. Book patients for reviews with GPs and the nursing team. Ensure timely follow-up and accurate record-keeping. Provide excellent patient support and communication throughout their care journey.

Essential Requirements:

Background knowledge of hypertension, diabetes, and other long-term conditions. Strong interpersonal skills with the ability to build rapport with patients and colleagues. Confident, organised, and able to work independently as well as part of a team. Good IT skills and attention to detail.

Desirable:

Previous experience in a healthcare setting. Understanding of primary care processes and patient pathways.
