We are seeking dedicated and compassionate Hub enablement workers to work at Ollerton and Ashfield Day Opportunities sites. You will be required to work across a district at different buildings. You will be responsible for delivering day opportunities activities and providing personal care and support to individuals within our hubs and beyond. You will be enthusiastic and flexible and be able to work across all service groups within a geographical area
This role is crucial in ensuring that we are providing exceptional day opportunities and support within community hubs, specialist services, and the community. Our mission is to enhance the quality of life for the people through innovative and compassionate care.
The Hub Enablement worker willprovide a range of assistance and support in response to the day-to-day personal, physical, and emotional needs of individuals, always respecting their dignity and being sensitive to age, disability, sexuality, gender, and cultural origin as described in care plans.
You will positively support individuals within a risk management framework and escalate any risks or unresolved difficulties to the Hub Leader/coordinator or Team Manager. They will contribute to the completion of care plans based on effective consultation with individuals, carers, specialist workers, and other relevant agencies.
You will comply with departmental health and safety policies and understand individual responsibility regarding legal health and safety requirements and follow departmental practice guidance, policies, and procedures.
You will deliver a wide range of day-to-day activities and support in accordance with care plans and our quality assurance framework and work with individuals on a one-to-one basis or in groups at a base or out in the community within a geographical area.
The hub enablement worker will support individuals in promoting, developing, and maintaining independence and in meeting their outcomes. Also contribute to the ongoing review and monitoring of care plans and risk assessments while ensuring quality standards are achieved.
You will use equipment as directed by the care plan, once appropriate training has taken place, and assist in handling individuals and inanimate objects.
You will respond appropriately to crisis and emergency situations and report any incidents as soon as possible.
You will engage in and attend staff meetings, undertake training, and contribute to the performance of day opportunities. You must ensure confidentiality of information in respect of records maintained and tasks undertaken within County Council Policy and relevant legislation.
You must:
Have a Professional care qualification or equivalent, or willingness to work towards standards equivalent to Level 2 Diploma in Adult Care and the Care Certificate.
Have basic level of ICT skills.
Excellent communication skills, both verbal and written.
Ability to work efficiently and effectively, both independently and as part of a team.
High level of punctuality and attendance.
You must have experience of working directly with adults, individuals or groups.
