Job Description

This is an exciting opportunity to work as part of the Integrated Discharge Team, supporting the inpatient teams to deliver the Home First Model of care , as a result we are looking to recruit a full time Band 6 Occupational Therapists to work as part of the team.



The philosophy of the Integrated Discharge Team is to provide holistic, highly skilled, integrated assessment by an experienced multi-disciplinary team to ensure patients are discharged from acute care in a timely manner, reducing delays and supporting the care closer to home agenda.



This exciting role will work as part of the Integrated Discharge Team, working closely with both the hospital and community teams to support and facilitate timely, functional assessments and discharges for patients to reduce length of stay and harms. Furthermore, facilitating safe, effective discharge to the most appropriate environment. This post will be based at the Royal Oldham working within the Integrated Medical Therapy Team.



There maybe the potential opportunity in the future for this post to be considered as part of the wider Band 6 rotation.



There will be a requirement to work weekends/bank holidays.



Work as part of the Integrated Discharge Team based with the Integrated Medical Therapy Team at Royal Oldham Hospital



Work in line with the agreed strategic objectives of Oldham Care Organisation



Utilise high levels of clinical assessment and communication skills to support effective discharge planning, reducing unnecessary hospital stays and to improve flow through the acute hospital pathway.



Work autonomously within trust policies and professional practice and maintain accurate and comprehensive patient records.



Perform specialist assessment of patients and determine a clinical diagnosis, develop, implement, evaluate and modify individualised treatment programmes, ensuring appropriate pathways of care and communication via liaison and referral to other agencies as required. The patient will present with acute or chronic conditions, some of which may be complex.
