to find a dedicated

Occupational Therapist

to join their

Reablement Team

-- a service that plays a vital role in restoring confidence, independence, and dignity to people recovering from illness, injury, or hospitalisation.

This short-term placement is ideal for an experienced OT who is ready to make a tangible difference in the community.

About the role:

As a Reablement OT, you'll work directly with clients in their homes or other settings to design personalised, strengths-based care plans that reduce the need for long-term support. You'll collaborate with OT Assistants, support workers, and other professionals to deliver creative, goal-focused interventions that empower clients to regain independence.

Key Responsibilities:

Conduct strengths-based occupational therapy assessments in community settings Create and implement personalised reablement plans in partnership with clients and their families Recommend and oversee the use of specialist equipment and home adaptations Work closely with reablement support staff to coordinate short-term interventions Support clients with complex occupational needs and contribute to positive risk management Maintain accurate records and contribute to service reviews and development

What you'll need:

A recognised Occupational Therapy qualification (degree or diploma)

HCPC registration

Minimum 2 years of post-qualification experience Experience working in reablement, intermediate care, or adult social care Strong assessment, planning, and goal-setting skills Ability to travel locally -- a valid

UK driver's licence
