We have an exciting opportunity for a Social Work Lead across the Acute Care Support Team, working in our acute hospitals, supporting FLOW.The Acute Support Team supports Bed Management in our Organisation, seven days a week.We are looking for a full-time Social Worker to support patients in their pathway beyond the acute care. This role will link across the acute hospitals across the Trust, supporting patients' discharges, supporting and unpicking barriers to discharge with other agencies.The candidate will need to have great networking skills, and be able to work across many teams in a collaborative and timely way. They will need to be able to chair meetings with outside agencies which do not work within our Organisation, and keep people to timescales and actions.This role will support quality of admissions and quality in managing resources, and will support patients in a timely way on their recovery journey.Main duties of the jobMonitoring all patients who have been in hospital for 20 days or more.Monitoring all Clinically Ready for Discharge (CRFD) patients.Support Ward Managers with barriers and blocks to discharge, with partners such as ICB and Local Authorities.Chairing meetings with partners to support actions for discharge and pathways.Managing daily updates of reports to the Associate Director of Capacity and FLOW.To build relationships with ICB.To build relationships with Local Authorities in Hampshire and IOW.Advise teams on Social Work interventions with Social Work expertise.To support with complex and 'super stranded' or hard to place patients in their discharge plans.To build relationships with Ward Managers, Matrons and Community Teams across the Trust.It is important to have an understanding of the costs involved in patient care and delays.About usHampshire and Isle of Wight Healthcare NHS Foundation Trust is a newly established organisation, bringing together expertise from across mental health, learning disabilities, community, and physical health services.
