Job Description

We have an exciting opportunity for a Patient Services Coordinatorto join our Elective Care team based at the Manor House in Sheffield on a full time basis.

The post-holder has lead responsibility for the smooth, seamless co-ordination of patient's outpatient pathway. They will be responsible for ensuring patients are treated in line with high quality health standards as detailed in contracts and service level agreements with agreed objectives, targets, quality standards, controls and resource constraints and providing an effective and efficient service to the clinical teams.

About the Role:



As a Patient Service Coordinator you will ensure all procedures and processes detailed within the Standard Operating Procedures are followed and adhered to at all times, including but not limited to:

Liaise with Consultants, Anaesthetists and other healthcare professionals, across sites to ensure clinical diaries are consistently booked with maximum utilisation and special/individual needs are established in a timely fashion to ensure no patient cancellations are on the clinic day and national targets are met. Schedule outpatient appointments within agreed dates ensuring that activity and performance targets for the organisation are met to ensure that no patient breaches national standards. Ensure the ePR is updated with all relevant data pertaining to the patients clinical pathway. Plan and organising patients' new and follow-up appointments and discharges by inputting the data the ePR. Contact patients to arrange appointments, scans and other clinical requirements as directed by consultants, contract managers and other healthcare professionals. Provide outpatient data information and act as the first point of contact for queries from clinicians / staff and patients. Undertake regular monitoring of clinics, review of clinic rules and profiles to make recommendations to ensure targets can be met in a timely fashion. Accurately update patients records with clinical outcomes ensuring onward referrals to other organisations or departments are delivered and received in a timely manner. Communicate clearly and effectively at all times with patients regarding their bookings, as well as with clinicians and operational teams to ensure a smooth and positive patient experience. Act as first point of contact for Trust enquiries, providing timely and accurate responses to colleagues in order to ensure patients are moved along their clinical pathway. Act as first point of contact for patient complaints and compliments, seeking resolution of issues where possible and escalating to line managers if needed. To be actively involved in reducing waiting lists/times, be aware of any possible breaches and ensure full delivery of waiting times/lists strategy. Seek and allocate consultant availability on a monthly basis to ensure clinical capacity is maintained. Procedure outpatient rota's for review by colleagues responsible for clinical staffing. Maintain an up to date account of Consultant and team leave. Work collaboratively with Trust colleagues to ensure consultant surgeons are available for theatre lists and arrange pre-operative assessments where required. Provide support and cover for the Patient Services Administrator during absences or busy periods. Ensure that organisational processes are adhered to at all times. Provide cover for annual and sickness leave within the wider team as directed by the Service Manager. Deal effectively and promptly all telephone enquiries from key contacts including GP's, liaise with internal and external sources to resolve any problems, queries or complaints. Deal with medical representatives and other professionals that attend the office to resolve any problems, queries, complaints or requests for appointments. Participate in team meetings and be actively involved in generating ideas for service improvement, and participating in any improvement projects as and when required. Ensure patient confidentiality is maintained at all times, as per Pioneer Healthcare policies. Comply with all policies, procedures and working standards identified by Pioneer Healthcare. Ensure that the reputation of Pioneer Healthcare is maintained in a positive way at all times. Any other duties deemed appropriate for the role.
